If you are considering having breast enlargement surgery, you should consider carefully the type of implant you are going to have, as both have their merits. There are actually two types of breast implants in use today. The first type consists of silicone gel, and the second is filled with saline solution. Both have advantages and disadvantages.
For example, the main advantage of silicone gel implants is that aesthetically they look better and more natural. They are also softer, which means they cannot normally be distinguished from real breasts, unless they develop a capsular contracture- an immune-system reaction to foreign bodies.
Silicone gel-filled implants comprise an outer layer constructed from several thinner layers of elastic silicone. They are then filled with silicone gel, which is clear and cohesive.
The advantage of saline implants is the fact that they are very unlikely to develop capsular contractures; and if they do, they are more likely to develop early on, meaning revision surgery is far less traumatic. Saline implants consist of a silicone outer layer. A valve in the implant allows for a sterilised saline solution to be introduced during surgery, and the size of the implant can then be altered as required.
When undergoing breast surgery, the implants can either be placed underneath the breast tissue or on top of the pectoralis muscle. They can also be placed slightly beneath the pectoralis major muscle as well, if required. Typically, the placement of the implant is dependent on the amount of existing breast tissue the patient already has. For example, a patient with minimal breast tissue may have the implant placed underneath the muscle so that the extra layer adds to the size of the cleavage.
During the breast enlargement procedure, the implant can be inserted through an incision in the crease underneath the breast (known as the infra-mammary fold). The incision is typically 3-6 cm in length, although this will depend on the size of the implant. It is also possible to insert the implant through an incision around the areola, known as peri-areola incision. The benefit of having an infra-mammary incision is the fact that the breast will naturally hide the scar, whereas there may be some visible scarring on the areola. This, however, will naturally fade with time. Another option is to have the implant inserted through the armpit. This is particularly beneficial for patients who prefer to have no scarring on the breast whatsoever and also for women who are Asian or black, as it reduces the likelihood of visible keloid scarring.
Breast Surgery is a serious undertaking, and one which should never be pursued without full consultation with a qualified and experienced consultant surgeon.
