Accident and Emergency Departments are probably the busiest place in a hospital, with a new patient arriving on average every ten minutes. * Wrongly diagnosed heart attacks;
* Failure to arrange scans or tests;
* Failure to properly interpret scan or test results;
* Delay in diagnosis.
